Position Title: REACH Community Liaison
Reports To: REACH Program Coordinator
The REACH Community Liaison (RCL) assists with planning and implementing programming for The Wellness Coalition’s Racial and Ethnic Approaches to Community Health (REACH) cooperative agreement. This includes chronic disease prevention/management interventions, including health care access, insurance enrollment, healthy food access, tobacco cessation, and chronic disease self-management education. The RCL works with faith-based and community organizations in the River Region to educate organizations on REACH objectives. The RCL also assists partners in providing healthy food access interventions. The RCL also provides technical assistance to community organizations and coordinates and provides administrative support for other evidence-based wellness projects. The RCL assists the REACH Program Management Team in implementing the REACH Work Plan, ensuring data collection for program evaluation, and preparing reports for submission to the Centers for Disease Control and Prevention.
Essential Duties and Responsibilities:
- Become familiar with evidence-based community health interventions outlined in the REACH Work Plan, including Wellness Case Management, Healthy Food/Health Care Access, Health Insurance Enrollment, Living Well Alabama, Tobacco and others.
- Assist in the planning and implementation of evidence-based interventions for healthy food access, tobacco cessation, and community and clinical linkages in River Region communities as outlined in the approved REACH Work Plan.
- Assist in the recruitment and development of new community sites for provision of services including education about interventions.
- Assist in the support of REACH River Region community organizations’ activities related to policy, systems, and environmental change efforts.
- Attend community meetings to provide technical assistance with grant implementation and outcome measures and to ensure that all activities coincide with the activity timeline.
- Develop partnerships with community agencies to leverage existing resources and increase capacity for sustainable programs.
- Make presentations about REACH resources and the REACH program mission and distribute materials to community groups, agencies, businesses and health care providers.
- Make site visits and obtain individual reports from community organizations who have received sub awards to ensure that REACH objectives are being achieved according to the agreement and guidelines.
- Support and participate in community-based healthy lifestyle campaigns and events throughout the year.
- Actively assist community volunteers in health insurance outreach and enrollment activities.
- Ensure that program evaluation data are collected from sites and submitted accurately and in a timely manner.
- Assist in preparation of required progress reports.
- Attend Program Update meetings and provide updates on progress of program components.
- Assist REACH Program Coordinator with other functions as required.
Qualifications:
- Must have a Bachelor’s degree in a health-related field OR training as a Community Health Worker and/or significant job-related experience.
- Applicants should be knowledgeable of community health interventions and resources within the River Region.
- Applicant must be able to communicate effectively in written and spoken word, able to work independently, computer literate and well organized.
- Ability to work a flexible schedule, including some nights and weekends.
- Must possess a valid driver’s license.

Compensation:

Salary: $36,000.00 – $40,000.00 per year
