Position Title: Community Health Workers (COVID-19)
Reports To: CHW Program Coordinator
This position provides community-based health information and support in response to the COVID-19 pandemic in the City of Montgomery. This team member will strengthen current COVID response efforts in Montgomery by conducting educational household and group outreach regarding COVID-19 prevention and transmission, pre-existing health condition management, and access to healthcare. CHWs will promote testing and vaccination events, facilitate referrals to health services, and conduct individual needs assessments to link COVID-affected community members to social support services. This position will conduct community outreach and household visits but does not require a clinical background and will not conduct clinical activities such as performing tests and screening. This position will work 40 hours per week for 11 months with possible extension.
Essential Duties and Responsibilities:
- Act as a bridge between the community, healthcare providers, other community organizations, and the local Department of Public Health to bring health information to households.
- Share feedback from the community to promote a coordinated COVID-19 response effort.
- Provide COVID-19 information and health education through household visits and community engagement.
- Promote COVID-19 testing and vaccination events within the community, promote the importance of COVID testing and vaccination, and provide material about supportive resources available to COVID positive individuals.
- Connect COVID positive individuals to social support resources in order to ensure a more feasible isolation and quarantine.
- Link community members to relevant clinical and social resources through the referral protocols.
- Support patients in the community with pre-existing conditions by providing healthcare referrals.
- Perform basic data collection during community outreach.
- Work with program consultants to utilize expertise in program implementation..
- Work with Wellness Navigators to facilitate insurance enrollment, chronic disease self-management education, and other TWC services.
- Participate in training and professional development with TWC staff, partners and volunteers regarding COVID-19.
- Assist in providing data for preparation of Community Health Worker Program evaluation reports.
Qualifications:
- Individual must be 18 years or older, must have a valid driver’s license, and preferably reside in the Montgomery area.
- Must have the ability to effectively communicate COVID-19 health-related information and resources using a variety of methods.
- Successful candidates/nominees must have a passion for serving the community and promoting health for all.
- CHWs must be from and/or intimately familiar with the communities they serve, currently operating as a trusted local leader or possess a keen ability to build trust with others.
- Candidates/nominees should have strong verbal/written communication and organizational skills, be emotionally mature, and have a collaborative spirit.
- Candidates/nominees must bring intrinsic cultural competency, resourcefulness, and empathy to the role.
- The successful candidate/nominee must be flexible, computer literate, and capable of managing a variety of activities and projects.
Education and Experience:
- Bachelor’s degree required in Accounting or Finance.
- Certified Public Accountant preferred.
- Five years of experience in non-profit accounting preferred, or ten years accounting experience.
- Case Management: 1 year (Preferred)
Beneficial Experience:
- Experience in health/social service field or community advocacy is beneficial.
- Fluency in a language other than English is beneficial.
