  • Mutual Respect. You will have an equal say and make decisions about your own health.

Do You Qualify for a Medical Home?

You may qualify for access to medical homes and other healthcare resources if you have little or no health insurance, use the emergency room for everyday needs, or have one or more chronic conditions, including (but not limited to):

Arthritis

Fibromyalgia, gout, psoriatic arthritis, etc.

Diabetes

Type 1 and type 2, as well as prediabetes

Heart Disease

High blood pressure, hypertension, and more

Lung Disease

Asthma, bronchitis, COPD, and more

Mental Illness

Alzheimer’s, dementia and other conditions

Obesity

Overweight leading to more conditions

Get Free Support With Wellness Case Management

Wellness Case Management is a service that can help you find a doctor and link you with medications and other services you need to manage a chronic disease. Your Wellness Case Manager can also help you set wellness goals and help you work to reach them.
